When Biscuit, a 13-year-old brown tabby, came in last spring, his owner apologized for “probably wasting my time.” “He’s just getting old,” she told me. “Sleeping more, picky about dinner, a little skinny along the back.” His bloodwork told a sharper story: stage 3 chronic kidney disease. I see some version of Biscuit nearly every week. Chronic kidney disease (CKD) affects up to 40% of cats over age 10 and as many as 80% of cats past 15, according to [1] — making it one of the most common diseases of aging cats and a leading cause of death in seniors. In dogs, estimates hover around half a percent to one percent of the population. Even geriatric dogs are diagnosed at a fraction of the rate of geriatric cats. So why does one species get devastated by a disease that largely spares the other? The answer is buried in evolution — and understanding it might change how you watch your own gray-muzzled cat.
Two Species, Two Very Different Kidneys
The kidney’s working unit is the nephron — a microscopic filtering tube. Cats are born with roughly 190,000 to 200,000 nephrons per kidney. Small dogs start with a similar number, but large breeds can carry well over a million per kidney. That difference matters enormously, because mammals can’t grow new nephrons. Everything you’re born with is everything you’ll ever have. A 90-pound Labrador simply has a bigger savings account to draw down over a lifetime than a 9-pound cat.
Then there’s ancestry. Cats descended from desert hunters who got most of their moisture from prey and evolved to produce extremely concentrated urine — up to a specific gravity of 1.080, among the highest of any mammal. The trade-off is a naturally weak thirst drive. Modern cats eating dry food and napping by the radiator don’t exactly rehydrate like their ancestors did, and many feline medicine specialists suspect this lifelong, low-grade dehydration stress quietly taxes the kidneys over 15 years.
Finally, cats are obligate carnivores. Their protein-heavy metabolism generates a constant stream of nitrogenous waste that the kidneys must filter, day after day, decade after decade. An omnivorous dog’s kidneys simply carry a lighter daily load. As one veterinary overview of [2] puts it, the kidneys handle waste removal, hydration, and electrolyte balance all at once — and in cats, all three systems are running closer to their limit from day one.
The Silent Filter: Why Symptoms Arrive Late
Beyond filtering waste, kidneys regulate hydration, balance electrolytes, activate vitamin D, help control blood pressure, and produce the hormone that tells bone marrow to make red blood cells. That’s a long job description — and here’s the cruel part: kidneys have so much built-in reserve that roughly two-thirds to three-quarters of function is gone before a cat looks sick. By the time symptoms appear, the damage is irreversible. That’s what I mean by the silent filter.
The earliest signs are so subtle that owners routinely blame them on age:
- Bigger clumps, more often. A client with a cat named Mabel once showed me a photo of clumps “the size of golf balls” in the litter box. That’s polyuria — excessive urination — and it’s often paired with noticeably increased drinking.
- Weight loss along the spine. Muscle wasting over the backbone is usually the very first physical change, and it’s easy to miss on a fluffy cat.
- An ammonia-like odor on the breath. Uremic toxins produce a distinct chemical smell. Owners frequently mistake it for dental disease — and many senior cats have both, so mention any odor change to your vet rather than assuming it’s teeth.
- A dull, unkempt coat (grooming takes energy cats no longer have), declining appetite, occasional vomiting, constipation, or seeking out warm spots because CKD cats tend to run cold.
- Pale gums, which can signal the anemia that develops when kidneys stop signaling the bone marrow properly.
The Bloodwork That Catches It Early
For decades, the standard kidney marker was creatinine — a waste product that rises in blood when filtration drops. The problem: creatinine often doesn’t climb until about 75% of kidney function is already gone. A newer test called SDMA (symmetric dimethylarginine) can flag trouble after roughly 40% function loss, which in a slowly progressing disease can mean catching it years earlier.
I’ll admit something here: I’ve changed my mind about SDMA. Early on, I worried it overdiagnosed healthy older cats — labeling a 14-year-old “diseased” over a mildly elevated number. Then I started watching cats like Clementine, an 11-year-old dilute tortoiseshell whose SDMA bumped slightly on a routine panel with no symptoms at all. We caught her at stage 2, started dietary management, and three years later she’s still stable and sassily supervising her owner’s kitchen. Early detection didn’t scare her family — it bought her time.
A proper senior screen includes:
- Bloodwork with SDMA, creatinine, and BUN — a baseline around age 7, then twice-yearly panels for cats 10 and older.
- Urinalysis — a urine specific gravity below about 1.035 in cats means the kidneys are losing their concentrating ability, often before blood values shift. A protein-to-creatinine (UPC) ratio checks for protein leakage.
- Blood pressure. Hypertension both results from CKD and accelerates it, and it’s treatable.
If CKD is diagnosed, your vet will assign an IRIS stage (1 through 4) — the international staging standard. Median survival runs roughly two to three years at stage 2, six months to two years at stage 3, and weeks to months at stage 4. Those are averages, not verdicts; plenty of cats outlive them.
Dogs Get Kidney Trouble Too — Just a Different Kind
When dogs’ kidneys fail, it’s usually sudden rather than silent. Acute kidney injury in dogs is dominated by toxins and infection: grapes and raisins, human anti-inflammatory painkillers, antifreeze, and leptospirosis spread through contaminated water. Last winter I treated Gus, a four-year-old Labrador who hoovered a handful of raisins off the counter — he needed days of intensive fluids but walked out fine, something that would have been unlikely had anyone waited for “symptoms to develop.”
Dogs also carry their own hereditary kidney diseases, concentrated by generations of selective breeding: Samoyeds, Bull Terriers, Bernese Mountain Dogs, and English Cocker Spaniels are among the breeds with recognized inherited nephropathies that show up in young dogs, not seniors. Geriatric dogs absolutely can develop CKD — but it’s far less common, and it’s more often secondary to another problem than the primary aging disease it is in cats.
Living With a Leaky Filter: What Actually Helps
The diet question
The single strongest evidence for extending survival in feline CKD isn’t a drug — it’s phosphorus restriction, typically through a therapeutic renal diet prescribed by your vet. Two honest controversies, though: first, the popular claim that dry food outright causes CKD remains unproven; wet food appears potentially protective in studies, but causation isn’t established. Second, aggressively restricting protein in an obligate carnivore risks the very muscle wasting we’re trying to prevent. Modern feline medicine focuses on phosphorus first. A 2016 study of [3] identified age, low body weight, and periodontal disease as risk factors, with wet food possibly protective — though, as the researchers note, thinness may be an early effect of the disease rather than a cause.
Beyond the food bowl
Depending on stage, tools include phosphate binders, subcutaneous fluids given at home (easier than it sounds — most cats tolerate it beautifully), blood-pressure medications like amlodipine, and telmisartan for protein-losing kidneys; a 2023 trial even suggested telmisartan may slow progression in cats without proteinuria. Newer appetite-support medications specifically approved for CKD-related weight loss in cats are helping cats keep muscle on. Every dose and combination should be tailored by your veterinarian — please don’t adjust anything on your own.
Small habits that matter
- Weigh your senior cat monthly on a kitchen scale and log it. A few ounces of drift tells you more than any eyeball.
- Photograph litter clumps weekly. Size trends are data.
- Never give human pain medication — a single ibuprofen-sized dose can hospitalize a cat.
- Keep up with dental care; the gum-disease connection keeps showing up in the research.
Biscuit’s owner left that appointment shaken but armed. Eight months later he’s two pounds of lost ground regained on a renal diet, and she tracks his weight on a sticky note on the refrigerator. The deeper lesson, though, belongs to all of us who love animals we’ve shaped through breeding: those same breeding choices concentrated kidney-disease genes — polycystic kidney disease once affected roughly a third of all Persians before DNA testing allowed responsible breeders to screen it out of bloodlines. Supporting breeders who test, and choosing adoption when we can, is conservation at the level of the gene pool. And for the cat already on your couch: the silent filter asks for so little — a twice-yearly blood draw and a watchful eye on the litter box. It’s how we finally give it a voice.
