I’ll never forget the afternoon a client rushed a sweet, four-year-old indoor tabby named Oliver into my clinic. Dogs are what we call the definitive host. This means the parasite considers the canine body a perfect home. The worms thrive, mate, and multiply. A dog can harbor dozens or even hundreds of these foot-long parasites, and their lifespan in a dog is a staggering 5 to 7 years.

When a heartworm dies inside a dog, the dog’s body can usually absorb the debris over time, especially with veterinary treatment. But when a heartworm dies inside a cat, it triggers a severe, acute inflammatory reaction in the lungs. The feline immune system goes into absolute overdrive, essentially causing the lungs to attack themselves. This condition is frequently fatal, and unfortunately, sudden collapse or sudden death is often the very first and only symptom the owner sees.
Spotting Feline Heartworm Symptoms: The HARD Reality
In dogs, the symptoms of heartworm are usually relatively predictable: a soft, persistent cough, fatigue after exercise, and gradual weight loss.
Feline heartworm symptoms are vastly different and notoriously misleading. In veterinary medicine, we call this presentation Heartworm-Associated Respiratory Disease, or HARD.
If your cat is infected, you might see:
- Coughing, wheezing, or asthma-like attacks. I’ve seen many cats misdiagnosed with feline asthma or allergic bronchitis when the real culprit was a dying heartworm.
- Unexplained vomiting. This is often mistaken for hairballs, but it usually isn’t associated with eating.
- Lethargy, decreased appetite, and weight loss.
- Sudden collapse. In severe cases, this is the first sign that something is terribly wrong.
The Diagnostic Challenge
Detecting heartworm in cats is incredibly difficult, and misdiagnosis rates are high. The standard antigen tests we use for dogs only detect a specific protein found in adult female worms. Remember how I said cats usually have only one worm? Statistically, that single worm is often male, or it hasn’t reached adulthood. This means standard tests frequently return false negatives.
To diagnose a cat, we have to play detective. I use a combination of antibody tests (which show if the cat’s immune system has ever encountered the parasite), clinical signs, and crucially, echocardiograms. There is a growing push in the veterinary community to use these ultrasounds as a primary diagnostic tool in cats precisely because blood tests are so unreliable.
The Treatment Divide: A Cure for Dogs, A Void for Cats
This is where the disparity between cats and dogs becomes truly heartbreaking.
Dog Heartworm Treatment
For dogs, we have an established, FDA-approved dog heartworm treatment protocol. It involves a series of injections using a drug called melarsomine, which contains arsenic. I’ll be honest with you—this treatment is rigorous, expensive, and requires months of strict cage rest. You have to keep the dog incredibly calm so their heart doesn’t pump too hard and push dead worm fragments into the lungs, which can cause a fatal blockage.
Treating a dog is hard on the pet and the owner. But it works. It is highly effective at killing the adult worms so the dog can fully recover. (Note: Never attempt to treat heartworm without consulting your vet, as the sudden death of many worms requires strict medical management).
The Feline Treatment Void
Now for the bad news: there is no approved drug treatment for heartworm disease in cats. None.
Because killing the worms causes that fatal lung inflammation, giving a cat the canine drug would almost certainly be fatal. If a cat is diagnosed, our only option is supportive care. We use steroids to suppress the immune system and dampen the inflammation, bronchodilators to keep the airways open, and oxygen therapy if they’re struggling to breathe.
Basically, we have to wait for the worm’s natural lifespan to end, hoping the cat survives the inevitable die-off. It’s a helpless feeling for a veterinarian and a terrifying experience for a pet owner.
The Indoor Cat Myth and Climate Realities
If you live in a warm, humid climate, you probably already know that mosquitoes are a year-round nuisance. But recent data from the American Heartworm Society shows heartworm incidence is migrating northward. Changing climates, increased mosquito resistance, and the relocation of rescue dogs from high-incidence states to low-incidence states are spreading the disease to entirely new areas.
But what if your cat never goes outside?
I hear this all the time: “Dr. Sarah, my cat is strictly indoors. She doesn’t need prevention.”
I used to nod and understand the logic. After all, how often does an indoor cat really encounter a mosquito? But the research changed my mind. Studies show that roughly 25% to 30% of cats diagnosed with heartworm disease are strictly indoor cats. Mosquitoes easily slip through open doors when you’re bringing in groceries, or squeeze through tiny tears in window screens.
Why Heartworm Prevention is the Only True Shield
Because feline heartworm disease cannot be safely treated, heartworm prevention is not just recommended—it is your cat’s only absolute defense.

The American Heartworm Society currently emphasizes a “Think 12” approach: test dogs every 12 months and give preventatives 12 months a year. They strongly urge the same vigilance for cats, regardless of the local climate.
