He was eating a popular, expensive, grain-free diet that Sarah had chosen because she wanted to feed him the absolute best. Like so many dedicated pet owners, she had fallen for the marketing myth that “ancestral” or grain-free means healthier. What Exactly is Dilated Cardiomyopathy (DCM)?

Before we dive into the food bowl, we need to talk about the heart. Dilated Cardiomyopathy (DCM) is a severe, often fatal disease of the heart muscle. In a healthy heart, the muscle is thick, strong, and flexible. It contracts tightly to pump blood out to the body and relaxes fully to refill. But in a dog with DCM, the heart muscle becomes stretched out, thin, and flabby—much like a balloon that has been inflated too many times. Because the muscle is so weak, the heart chambers enlarge (which is what “dilated” means), and the heart loses its ability to pump blood efficiently. This eventually leads to congestive heart failure, fluid building up in the lungs, and extreme fatigue.

Historically, we mostly saw DCM in specific large breeds that had a genetic predisposition, like Doberman Pinschers, Boxers, and Great Danes. But around 2018, cardiologists and general practitioners like myself started noticing a terrifying trend. We were seeing DCM in breeds that almost never get it—Golden Retrievers like Cooper, mixed breeds, and even smaller dogs. The one common denominator? Their diet.

The FDA Steps In: Connecting the Dots

When veterinarians across the country started reporting these unusual cases, the FDA’s Center for Veterinary Medicine (CVM) launched an ongoing,[1] between certain dog foods and canine DCM. The data they collected was staggering. By November 2022, the FDA had received 1,382 reports of DCM in dogs. To put that in perspective, the FDA typically receives a tiny fraction of that number for dietary issues.

When researchers analyzed the diets of these affected dogs, they found that more than 90% of the implicated diets were grain-free. But it wasn’t just the absence of grains that caught scientists’ attention. A staggering 93% of the reported diets contained high levels of peas or lentils.

Is it the Missing Grains, or the Added Legumes?

This is where the science gets really interesting, and it’s a hot topic of debate in veterinary nutrition. Is the problem the lack of grains, or is it the ingredients used to replace them?

Right now, the evidence strongly points toward the latter. To make a kibble stick together and meet certain macronutrient profiles without using wheat, corn, or rice, pet food manufacturers often rely heavily on legumes—like peas, lentils, and chickpeas—as well as potatoes. Researchers believe these ingredients may be interfering with the dog’s ability to absorb or synthesize taurine, an essential amino acid for heart health. I always explain it to my clients like this: imagine taking out a solid, well-tested load-bearing wall in your house (the grains) and replacing it with a stack of cheap, untested building blocks (the legumes). Eventually, the structure is going to weaken.

The “Ancestral Diet” Marketing Myth

If you walk down the pet food aisle today, you are bombarded with buzzwords: “grain-free,” “holistic,” “biologically appropriate,” and “ancestral.” Pet food marketing has brilliantly tapped into human nutrition trends. Because many humans are opting for gluten-free or low-carb diets, pet food companies implied that our dogs must need the same.

But dogs aren’t wolves. Thousands of years of domestication alongside humans means their genetics have changed. Modern dogs have evolved to digest starches and grains incredibly well. In fact, they possess specific genes (like the AMY2B gene) that produce amylase, an enzyme that breaks down starches right in the mouth and gut.

True food allergies in dogs are relatively rare, and when they do occur, they are almost always triggered by animal proteins like beef, dairy, or chicken. I’ve changed my mind on this over the years. Early in my career, I used to nod politely when clients told me they chose grain-free because their dog had “sensitive skin,” assuming it couldn’t hurt. Today, with the data we have, I actively steer my patients away from grain-free diets unless a dog has a formally diagnosed, rare medical condition requiring it. When you are choosing a diet for your dog or cat, look past the pretty packaging and the marketing buzzwords. Look for a statement on the bag from the Association of American Feed Control Officials (AAFCO) stating that the food is formulated to meet nutritional levels, or better yet, that it was established through animal feeding trials. Look for brands that employ a full-time, board-certified veterinary nutritionist and conduct peer-reviewed research on their actual products. You want to see high-quality animal proteins at the top of the ingredient list, supported by easily digestible energy sources like rice, oats, or corn.

At the end of the day, our companion animals rely on us completely to make the best choices for their health and welfare. The explosion of diet-associated heart disease is a stark reminder that good intentions can sometimes be led astray by clever marketing. By relying on evidence-based veterinary science rather than human food trends, we can ensure our pets stay by our sides, with strong, healthy hearts, for many years to come.
