Tommy was a twelve-week-old Labrador puppy whose owners brought him in because he had a potbelly and wasn't gaining weight the way they expected. He was eating well, playful enough, no vomiting. His breeder had said he was perfectly healthy.
The fecal float told a different story: a heavy roundworm burden. His owners went quiet when I showed them the eggs under the microscope — then a little pale when I explained those eggs had been shed into the yard, into the grass, and onto everything Tommy walked across. He was dewormed that day and had gained nearly a pound by his next visit two weeks later.
Intestinal worms are among the most common parasites in domestic dogs and cats. The disconnect between how ordinary they are and how alarming people find them never quite closes — but beyond the gross factor, these parasites cause real clinical disease.
The Major Players
Roundworms (Toxocara canis, Toxocara cati)
Roundworms are the most prevalent intestinal parasite in both dogs and cats. In puppies, they're nearly universal — Toxocara canis larvae cross the placenta before birth and also transmit through the mother's milk. Kittens acquire Toxocara cati primarily through nursing.
Adult roundworms live in the small intestine, where they feed on partially digested food. They're surprisingly large — adults measure 10-15 cm — and in heavy infections, especially in young animals, the worm burden alone can cause intestinal obstruction. I've had a few cases where puppies vomited live roundworms, which is genuinely startling for owners.
Eggs passed in feces become infective in the environment within 2-4 weeks and can persist in soil for years. Dogs and cats of any age can acquire roundworms by ingesting contaminated soil or by eating prey animals (rodents, birds) that carry encysted larvae in their tissues.
The zoonotic risk is real. Children who ingest Toxocara eggs from contaminated soil (playground dirt, sandboxes) can develop visceral larva migrans — larvae migrating through organs — or ocular larva migrans, which can cause vision loss. This is why deworming schedules for puppies and kittens begin as early as two weeks of age.
Hookworms (Ancylostoma caninum, Ancylostoma tubaeforme, Uncinaria stenocephala)
Hookworms are blood feeders. They attach to the intestinal wall, feed on blood, and move to new attachment sites — leaving behind bleeding lesions at each previous site. A heavy hookworm infection can cause severe, life-threatening anemia, particularly in puppies and kittens.
Transmission routes include ingestion of larvae from the environment, larval penetration through the skin (hookworm larvae in contaminated soil can burrow directly through the foot pads), and transmission through the mother's milk. Adult dogs pick up hookworms by walking on contaminated ground — which is why they're especially common in dogs housed on dirt runs or in warm, humid climates where larvae survive well in soil.
The zoonotic species can also penetrate human skin, causing cutaneous larva migrans — itchy, winding red tracks where the larva migrates through the skin surface. Walking barefoot in areas frequented by undewormed dogs is the classic exposure.
Whipworms (Trichuris vulpis)
Whipworms primarily affect dogs. They live in the cecum and large intestine, embedding their thin anterior end into the intestinal wall. Infections range from asymptomatic (light burdens) to chronic bloody diarrhea with mucus, weight loss, and debilitation.
What makes whipworms frustrating is diagnosis. The eggs are shed intermittently, so a single negative fecal test doesn't rule out infection. I've diagnosed dogs who'd had three negative fecals before the fourth one finally showed eggs. If a dog has chronic large-bowel diarrhea and negative fecals, I'll sometimes treat empirically for whipworms because the treatment is safe and the diagnostic false-negative rate is high.

Tapeworms (Dipylidium caninum, Taenia species)
Tapeworms are the worms owners actually notice. The proglottids — rice-grain-sized segments that break off the adult worm and pass in feces or crawl around the pet's perianal area — are visible and motile. They dry into small flat segments that stick to bedding and fur.
Dipylidium caninum is by far the most common tapeworm and is transmitted by fleas. The dog or cat swallows an infected flea during grooming, the tapeworm cyst inside the flea is released in the stomach, and a new tapeworm develops in the intestine. This means you can't solve a tapeworm problem without solving the flea problem.
Taenia species come from consuming infected prey — rodents, rabbits. Outdoor and hunting cats acquire these routinely.
Tapeworms are not detected by standard fecal flotation testing. The proglottids are the diagnostic clue — if you see them, that's the diagnosis. I've had clients bring in what they think is "white rice" on their cat's bedding, and it's tapeworm segments.
Detection and Testing
Fecal testing (fecal flotation and/or centrifugation) remains the primary diagnostic tool. A fresh stool sample is processed to concentrate parasite eggs, which are then identified under a microscope. The test is inexpensive and quick but has limitations: intermittent shedding means a single negative test isn't definitive, and tapeworm eggs are rarely detected by flotation.
Newer fecal antigen tests can detect proteins shed by worms even when eggs aren't present, improving sensitivity for some species. These are increasingly available and I use them for cases where I strongly suspect worms despite negative flotation.

Treatment
Deworming medications are species-specific and worm-specific:
Pyrantel pamoate is effective against roundworms and hookworms. It's the base ingredient in many puppy deworming protocols and over-the-counter dewormers.
Fenbendazole covers roundworms, hookworms, whipworms, and some tapeworm species. A broader-spectrum option that I reach for when multiple worm types are suspected or confirmed.
Praziquantel is the standard treatment for tapeworms. It's fast-acting and effective, but without concurrent flea control, reinfection is inevitable.
Milbemycin and moxidectin — ingredients in many monthly heartworm preventives — also provide ongoing protection against roundworms, hookworms, and whipworms. This is the prevention-as-treatment approach: consistent monthly use keeps these worms from establishing.
Treatment is straightforward for most cases. The complicated cases are heavy burdens in debilitated puppies who need supportive care alongside deworming, and chronic reinfection in contaminated environments.
FAQ
Can I see worms in my pet's stool?
Sometimes. Roundworms are visible — long, spaghetti-like worms. Tapeworm segments are visible as small white rice-like objects. Hookworms and whipworms are small and rarely visible to the naked eye. Absence of visible worms doesn't mean absence of infection, which is why fecal testing matters.
How did my indoor cat get worms?
Multiple routes. If the cat was ever outdoors — even briefly — it could have ingested contaminated material. Roundworm eggs persist in soil and can be tracked inside on shoes. Tapeworms come from swallowed fleas, and fleas can enter homes on clothing or other pets. And many kittens arrive already carrying roundworms from their mother.
Do I need to treat the whole household if one pet has worms?
If one pet is positive, there's a reasonable chance others in the household are exposed. I recommend testing all pets in the home and treating any positives. For roundworms and hookworms in a multi-pet household, I'll often recommend treating all dogs and cats as a precaution.
Can over-the-counter dewormers replace veterinary treatment?
Over-the-counter dewormers contain legitimate active ingredients (usually pyrantel), but they only cover roundworms and hookworms. If your pet has whipworms or tapeworms, an OTC product won't help. And without a fecal test to identify which worm is present, you're guessing. I'd rather know what we're treating and use the right drug.
