Nadia was an eight-year-old intact female German Shepherd whose owner had intentionally kept her unspayed — they'd read about hormone-related joint health and wanted to wait. She came in on a Tuesday evening lethargic and off her food. By Wednesday morning her ultrasound showed a uterus full of pus. We did emergency surgery that afternoon. Nadia made it, but it was a close call, and the surgery at eight years old was significantly riskier than a planned spay at two would have been.
The benefits of spaying and neutering are real and well-documented. So are some of the risks. The problem with most discussions is that they present one side as if the other doesn't exist. Neither extreme is accurate.
The Clear Benefits
Pyometra prevention in females. This one isn't subtle. Pyometra — a bacterial infection that fills the uterus with pus — affects roughly one in four intact female dogs who live past ten years old. It requires emergency surgery that's far more dangerous than an elective spay. I've lost patients to pyometra. I've never lost one to a routine spay. That math matters to me.
Mammary tumor risk reduction. Spaying before the first heat cycle reduces mammary tumor risk to under 1%. After the first heat, the risk climbs to about 8%. After the second, roughly 26%. By the time you're spaying a dog who's had multiple cycles, the mammary protection is minimal. Cats follow a similar pattern, and feline mammary tumors are malignant about 85% of the time — much higher than in dogs.
Elimination of testicular cancer and dramatic reduction in prostate disease. Neutered males can't develop testicular tumors. Benign prostatic hyperplasia, which affects most intact male dogs over five years old, essentially doesn't occur in neutered dogs.
Behavioral improvements. Roaming drops by roughly 90% in neutered males. Urine marking decreases in about half of dogs. Inter-dog aggression driven by testosterone diminishes significantly. Cats show even more dramatic behavioral shifts — intact male cats spray, fight, and roam compulsively, and neutering resolves most of this.
Population control. I work with rescue organizations, and the reality is blunt: there aren't enough homes. Every unplanned litter adds to a system that already euthanizes millions of animals annually.
The Legitimate Risks
This section is newer to the conversation, and I think it deserves honest treatment.
Joint disorders in large breeds. Research out of UC Davis, particularly their longitudinal breed-specific studies, showed that Golden Retrievers neutered before twelve months had notably higher rates of cranial cruciate ligament (CCL) rupture and hip dysplasia. Similar trends appeared in German Shepherds and Labrador Retrievers. The proposed mechanism: sex hormones influence growth plate closure, and without them, long bones may grow slightly longer than intended, altering joint mechanics.

Certain cancer risks. Here's where it gets uncomfortable to talk about honestly. Some studies have linked early spay/neuter to increased rates of hemangiosarcoma, lymphoma, and mast cell tumors in specific breeds. The Golden Retriever study, for instance, found that spayed females had a higher rate of hemangiosarcoma than intact females.
But context is everything. Cancer in pets is multifactorial. Genetics, diet, environmental exposure, and plain bad luck all play roles. Attributing a cancer case solely to a sterilization choice made years earlier oversimplifies a complex biology.
Urinary incontinence in spayed females. Between 5-20% of spayed female dogs develop hormone-responsive urinary incontinence, often years after surgery. It's manageable with medication, but it's a real and lifelong condition. Larger breeds are more commonly affected.
Metabolic changes. Both spayed and neutered pets have lower metabolic rates, leading to increased obesity risk if feeding isn't adjusted. This is less a surgical complication and more a management reality, but it's frequently overlooked.
Anesthetic risk. Any surgery under general anesthesia carries some risk. For healthy young animals, the mortality rate is well under 0.5%. For older animals or those with underlying conditions, it's higher but still low with proper monitoring.
Breed and Size Make the Difference
I used to give the same recommendation to everyone: spay or neuter at six months. My thinking has evolved.
For cats and small breed dogs (under 20 pounds adult weight), early sterilization around five to six months still makes excellent sense. The joint and cancer risk data that shifted the conversation mostly doesn't apply to these animals. The benefits are clear and the risks minimal.
Medium breeds occupy a gray area. I discuss individual circumstances — activity level, household situation, owner commitment to managing an intact animal.
Large and giant breeds are where individualized decision-making matters most. Waiting until twelve to eighteen months (or even later for giants) allows skeletal maturity and may reduce the orthopedic risks. But it means managing an intact large dog through adolescence, which comes with its own set of challenges and risks — including the possibility of an unplanned pregnancy.
A Word About the Research
I want to be transparent about something. Most of the studies driving timing recommendations have limitations. Many are retrospective (looking backward through medical records rather than following animals prospectively). Sample sizes for individual breeds are sometimes small. And the UC Davis studies, while valuable, focused on a handful of breeds — extrapolating their findings to all breeds requires caution.
We're getting better data every year. The Morris Animal Foundation's Golden Retriever Lifetime Study is following over 3,000 dogs from puppyhood through their entire lives. That kind of longitudinal data will sharpen our recommendations significantly.
Until then, I'm making my best clinical judgment based on imperfect evidence. I'd rather be honest about that than pretend we have all the answers.
My Personal Take
I spay and neuter most of my patients. I believe the collective benefits — health, behavioral, and population-level — outweigh the risks for the vast majority of pets. But I've moved from a rigid "six months for everyone" to a more nuanced approach that considers breed, size, and individual circumstances.
What I won't do is let a pet stay permanently intact without a plan. The lifetime risks of pyometra, mammary cancer, testicular issues, and behavioral problems are too significant to ignore.

My breeder says I should never spay or neuter. Who do I listen to?
I'd listen to your veterinarian, who can assess your specific pet's breed, size, health status, and your household circumstances. Some breeders have valid concerns about early sterilization in their specific breed — particularly large breeds with known orthopedic predispositions. But a blanket "never" recommendation ignores substantial evidence of health and behavioral benefits.
